What is it: 5 Principles for Problem Solving is a 5 step problem solving methodology for problem solving and corrective actions for non conformances.

Why use it: Is used to report results of sorts, repairs, investigations and corrective actions against a supplier. It helps their suppliers think through a problem to get clear root causes and solutions.

When to use it: Anytime a problem needs complete understanding and investigation

How to use it: 5 Principles for Problem Solving are:

1. Define the problem (5-Why's & 2How's)
Who - model & type
What - Symptom
When
Where
Why - Why is this a problem

How - How discovered?
How many - frequency of problem.

2. Identify the root cause
Q & A analysis - work toward finding root cause by repeating "Why?" at least 5 times. Investigate both Cause of Occurrence (usually "hard") and Cause of Non-detection ("soft" ).

3. Counter measures
Temporary & permanent counter measures. Think beyond fix: c/m sometimes creates new problems. Ask, "How will this effect other processes?"

4. Confirm effectiveness of counter measure(s)

5. Feedback/ feed forward.
Apply c/m to similar processes & similar problems. Incorporate counter measures into organization & management.
